[[TOC]] == Medical Records for Emergencies == *By: David Bond *Class: ITC 371 Personal Computer Applications *Instructor: Dr. Connie White *Semester: Fall 2013 == Introduction == The Medical Records for emergencies project will manage patient data during rescue and first aid operations. The data collected from patients will be used to help determine the level of injuries and recommend how best to treat the patients. == Project Description == The Philippines and surrounding areas have been devastated by a deadly typhoon on November 8th 2013. This project will collect data from injured patients while they are being treated. There are three zones that the injured patients will go through to be processed. 1.The first area is the Triage Zone. Patients will be photographed, classified, labeled with a RFDI tag and assigned a triage status. 2.The second area is First Aid and Treatment Zone. Patients will be tracked from their RFDI tag and personal data will be collected along with contact information. Vitals, relevant diagnoses, emergency interventions and lab data will be collected. 3.The third area is the Reporting Point at Hospital Admissions. Patients will be tracked from their RFDI tag and the patient will be registered and a report will be made. All of the patient’s data will be transferred to other medical record systems and a report with the patient's statistics will be created. == Requirements == *Patients will have a wrist tag-label with a readable (Barcode,QR,RFID). *Patients will have their photograph taken. *Patients will have a classification of (age, sex, race). *Patients will have their triage status determined. *Patients will be tracked from their tag-label. *Patients personal and contact data will be collected. *Patients will have their vital signs taken. *Patients will be given a diagnoses of their condition. *Patients will receive emergency interventions if their condition requires it. *Patients will receive emergency lab work if their condition requires it. *Patients will be registered in the computer system. *Patients will have a report generated with all relevant data collected. *Patients will have all data collected transferred to other medical record systems. *Statistics on all the patients will be updated. == User Case == *This describes how the doctor or staff member will interact with the patient being treated to collect data. 1. The doctor or staff on duty in the triage zone will collect data from the patient. The first step the doctor will determine the triage status of the patient and then take a photograph and input the patients name, age, race, sex and triage status into the computer. Then a RFID readable label-tag will be placed on the patient’s wrist. 2. The doctor or staff on duty will take the patient to the first aid and treatment zone. The doctor or staff on duty in this zone will scan the patient’s wrist- tag and collect personal and contact data along with vital signs from the patient. The doctor will then give a relevant diagnoses and classification of the patient. Any lab data or emergency interventions will be recorded at this time. The doctors will be able to track the patients from their wrist- tag. 3. The doctor or staff on duty in the reporting point of hospital admissions will scan the wrist-tag on the patients that enter this area; this will insure the correct patient is being treated. The patient will be registered for admission. All data collected from the patient will be transferred to other medical record systems and a report will be made. The statistics of the patient’s will be recorded. The doctor or staff can track the patient by scanning their wrist-tag. *This describes how the patient will interact with the doctor or staff member while being treated and giving relevant data. 1. The patient will enter the triage zone and will be greeted by a doctor or staff member. The patient will get their photograph taken and answer questions about their name, age, race and sex. The patient is given a RFID readable label-tag on their wrist. 2. The patient is taken to the first aid and treatment zone by a doctor or staff on duty. The patient gets their wrist scanned and then answers questions about their personal and contact data. The patient gets their vital signs taken, and they receive a diagnosis from the doctor or staff on their condition. 3. The patient is taken to the reporting point of hospital admissions by a doctor or staff on duty.
